Highly unlikely, and probably more difficult to find, but your description of the inlet and outlet ports is that of a 410 or 411 radiator.  Is this any help?  Not sure if the tab's holes for the mounting bolts are the same.  Maybe cannibilize a 410 or 411 upper and lower assembly and graft onto a 510 main radiator?

I buy generic radiators from Summit and then modify them as required.

 

A cheap aluminum radiator comes in many shapes and sizes from companies like Afco or BeCool. You can get one for less than $200 and a local radiator shop or fab shop can weld on any size inlet/outlet, move the filler neck and add things like threaded bungs very easily. Usually for less than $100. Mounting is the biggest issue, but you can get generic flanges on a custom radiator or you can mount them using rubber bushings like Ford and Chevy have done for decades.

 

Think outside the box.
